Why are cancers missed in lymph nodes?

Limitations of imaging: While high-resolution ultrasound is excellent at spotting suspicious nodes, it can't distinguish cancer from non-cancerous (reactive) nodes just by looking at them.
Location matters: Ultrasound is less sensitive at detecting spread in the central neck area compared to the side because of the presence of the thyroid gland itself.
 
 
 
 
How doctors manage this
Routine lymph node dissection: Many surgeons routinely remove lymph nodes around the thyroid during surgery for known cancer. This is done to find and remove microscopic cancer that might not have been seen on imaging.
Fine-needle aspiration (FNA): If a lymph node looks suspicious, doctors will often perform an FNA to get a tissue sample for a definitive diagnosis. Using a special technique, thyroglobulin or calcitonin can be measured in the needle's fluid washout, which can be a very sensitive indicator of cancer even if the cells themselves are indeterminate.
